Overview
The Alzheimer's Network of Oregon is a nonprofit network of experienced volunteers offering support and resources to persons with memory loss, their families and caregivers through the Memory Cafe. Information gathered for members is for Alzheimer's Network purpose only.
Collection, use and disclosure of personal information
Alzheimer's Network requires participants name, along with the name of their chosen company (i.e., family members and caregivers.) If more than one person will be joining the participant, their information will also be needed. We will need to know who of the family/caregiver will be the point of contact. That person needs to share address, phone number and email. Those pieces of information are for Alzheimer's Networks eyes only. It is used solely to contact about Memory Café dates, locations, changes, and information regarding other events or important information. Alzheimer's Network will not be flooding point of contact with spam. The information given will not be public and can be changed at any time.
